Responsibilities

• Provide in-home nursing care by assessing, developing, implementing, and evaluating care plans tailored to each patient's needs.
• Follow physician orders for patient visits, ensuring documentation reflects compliance and progress toward discharge goals.
• Deliver holistic, safe, and effective care while maintaining high standards of nursing practice.
• Conduct thorough assessments, update care plans as patient conditions change, and provide medication reconciliation.
• Collaborate with interdisciplinary teams, physicians, and managers to coordinate care and resource referrals.
• Serve as a clinical supervisor for home health aides, conducting evaluations and ensuring quality care delivery.
• Maintain compliance with payor requirements, secure authorizations as needed, and document all activities accurately.
• Educate patients and caregivers on treatment plans and document teaching strategies.
• Adhere to all organizational standards, safety protocols, and ethical codes of conduct.

Qualifications Required

• Associate's Degree in Nursing or a Diploma in Nursing.
• Minimum of 6 months of RN experience (or 1 year of LPN experience); home care or hospice experience is preferred.
• Proficiency in basic computer skills.
• Active NJ State Professional Registered Nurse License.
• AHA Basic Life Support (BLS) Certification.

Preferred

• Bachelor of Science in Nursing (BSN).
