About Us Talbot Health Services is a CARF-accredited, OhioMHAS-certified behavioral health and addiction treatment agency serving communities across Ohio. Our Toledo campus offers a full continuum of care: women's residential treatment, partial hospitalization (PHP), intensive outpatient (IOP), and outpatient services, alongside psychiatry, medication management, day treatment, and community-based case management. Clients move through a structured, phased treatment model — which means you see real progress happen, from a client's first week in induction to the day they step down to monthly ongoing care. The Role As a Counselor at our Toledo location, you'll carry a caseload across our levels of care and be the clinical anchor of each client's recovery. Reporting to the Director of Clinical Services, you will: Facilitate group counseling sessions and weekly individual sessions within our PHP, IOP, and outpatient phase structure Complete diagnostic assessments and build individualized, trauma-informed treatment plans Guide clients through phase advancement, and help those who are struggling build a plan for success rather than fall out of care Coordinate with our psychiatrists, medical staff, and case managers as one care team Document to CARF and OhioMHAS standards, maintaining confidentiality under 42 CFR Part 2 and HIPAA Who We're Looking For Active Ohio licensure in good standing: LSW, LPC, LCDC II, or LCDC III Bachelor's degree in social work, counseling, psychology, or related field Preferred: independent licensure (LISW, LPCC, LICDC), master's degree, and experience in SUD or community behavioral health Above all: someone who believes the people we serve deserve dignity, structure, and a real shot at recovery What We Offer Full-time position with competitive pay and a comprehensive benefits package, in a growing agency where advancement is real — many of our leaders started in direct care roles.
