HIV Medical Case Manager (LVN/RN) - Care Navigator
Prism Health North Texas, Inc.
Prism Health North Texas, Inc. is seeking a Medical Case Manager to provide treatment adherence and client-centered services for individuals living with HIV. You will coordinate care, develop care plans, and ensure the delivery of quality services while being sensitive to the needs of clients. The role requires strong communication skills, medical knowledge of HIV, and teamwork. Preferred qualifications include a Bachelor’s or Master’s degree in nursing, along with at least 2 years of relevant experience.
