Employment Type: Part‑Time/ Per‑Diem (Flexible Scheduling Available) Compensation: Competitive Pay (starting at $50.00 per hour) What You Will Do: Conduct Start of Care (SOC) visits to complete comprehensive clinical assessments and develop individualized, physician‑directed care plans Perform skilled nursing visits and ongoing reassessments to monitor patient condition and adjust care plans as needed Provide hands‑on skilled nursing care , including wound care, medication management, injections, and chronic disease management (for skilled nursing positions only) Deliver patient and caregiver education on diagnoses, medications, and preventive care to reduce hospitalizations Collaborate with physicians and interdisciplinary team members to ensure coordinated, high‑quality care Maintain accurate and timely clinical documentation in compliance with state regulations and agency standards Supervise and delegate care to LPNs and Home Health Aides to ensure quality and adherence to the plan of care What You Will Have: Active Registered Nurse (RN) license in the state of practice Experience in home health, skilled nursing, or a related healthcare setting preferred Strong clinical assessment and critical thinking skills Knowledge of home health regulations and documentation standards Excellent communication and interpersonal skills Ability to work independently and manage time effectively A passion for providing compassionate, patient‑centered care Why Join Us: Competitive pay with flexible scheduling options Opportunities for career growth and advancement Supportive leadership and team environment Make a meaningful impact in patients’ lives every day Weekly pay 401k with company match Health, Dental, and Vision insurance for full‑time employees + additional wellness benefits Paid time off and sick time Attend Home Care is an Equal Opportunity Employer.
