Physical Therapist, PT
Fountain Care at Sunset Hills is seeking a compassionate and skilled Per Diem Physical Therapist to join our dedicated rehabilitation team. If you are passionate about helping patients regain mobility, improve independence, and enhance their quality of life, we encourage you to apply.

Job Responsibilities
- Evaluate residents and develop individualized treatment plans.
- Provide physical therapy services in accordance with physician orders and established standards of care.
- Monitor patient progress and modify treatment plans as needed.
- Educate patients, families, and caregivers on therapeutic exercises and mobility techniques.
- Collaborate with physicians, nurses, and interdisciplinary team members to ensure optimal patient outcomes.
- Complete timely and accurate clinical documentation.
- Maintain compliance with all federal, state, and facility regulations.
Qualifications
- Current Missouri Physical Therapist license in good standing.
- Graduate of an accredited Physical Therapy program.
- Excellent communication and interpersonal skills.
- Experience in skilled nursing, rehabilitation, or long-term care is preferred but not required.
- Ability to work independently and as part of a multidisciplinary team.
