Responsibilities
- Perform initial evaluations and ongoing assessments of home health patients to determine functional status and rehabilitation needs
- Develop and implement individualized plans of care (POC) for physical therapy, with measurable goals and regular re-evaluation
- Deliver evidence-based PT interventions to improve mobility, strength, balance, endurance, and activities of daily living in patients' homes
- Monitor patient progress, adjust treatment plans, and document outcomes in the appropriate electronic health record system
- Educate patients and family caregivers on exercises, safety, and home modification strategies; coordinate with the broader care team for comprehensive discharge planning
- Ensure compliance with home health documentation standards, including HCHB and OASIS data entry, and uphold patient safety and infection control protocols

Requirements
- Doctor of Physical Therapy (DPT) degree required
- Active Physical Therapist license in California or eligibility to obtain licensure
- Minimum 1 year of home health experience
- Knowledge of Home Health Clinical systems and documentation, including HCHB and OASIS
- AHA BLS (Basic Life Support) certification required
- Ability to work in a home health environment, with day shift availability (Mon-Fri)
- Strong communication and interpersonal skills; reliable transportation within the service area

Education
- Doctor of Physical Therapy (DPT) degree required

Experience
- At least 1 year of home health experience recommended

Certifications
- AHA BLS required
