Job Description

Join our team as a Registered Nurse Case Manager at a Level II trauma hospital with 440 beds. This role focuses on working with a triad model as part of a collaborative team including Social Workers and Certified Medical Assistants. The RN Case Manager will handle initial assessments and discharge evaluations, requiring proficiency in Interqual.

Responsibilities

• Conduct initial assessments and discharge planning for patients.
• Collaborate with the multidisciplinary team to develop and implement care plans.
• Maintain effective communication with healthcare teams and patients to ensure continuity of care.
• Utilize Meditech and other documentation systems for managing patient information.

Required Experience

• Active RN license in Florida or a compact state.
• 2-4 years of experience in case management within an acute care facility.

Certifications/Licensure

• Current RN license.
