What You'll Do

• Provide compassionate, high-quality nursing care to residents.
• Administer medications and treatments according to physician orders and facility policies
• Monitor residents' health status and report changes promptly to the RN or appropriate healthcare provider.
• Perform nursing assessment and document care accurately
• Assist with wound care, treatments, and other nursing procedures with the LPN Scope of practice.
• Collaborate with RNs, physicians, CNA's therapists, and other members of the healthcare team
• Maintain accurate and timely clinical documentation.
• Follow infection-control procedures and all facility safety policies.
• Promote residents' dignity, comfort, independence and overall well-being
• Communicate effectively with residents, families, and members of the interdisciplinary care team.

Requirements

• Current and valid New York State LPN License.
• Graduate of an accredited Practical Nursing program
• Previous experience in skilled nursing, long-term-care, rehabilitation, or a similar healthcare setting is preferred
• Strong clinical assessment, communication and organizational skills
• Ability to work effectively in a fast-paced healthcare environment
• Reliable, compassionate, professional, and patient-focused.
• Ability to work independently and collaboratively as a part of multidisciplinary team.
• Must be able to meet all facility onboarding, background-check, and heath-screening requirements.
