Day Program LSC: Full-time Location: West Haven, CT, US Salary Range: $17.75 To $22.75 Hourly The West Haven Community House is currently seeking to hire dedicated Life Skills Coaches to work with our wonderfully unique individuals in our DDS funded Day Program. The LSC is expected to work independently throughout the shift, flexing the daily schedule to meet the needs of the individuals being served. The LSC works as part of a team committed to meeting the needs of individuals in the areas of ADL, socialization, behavior management, personal safety, and goal achievement. The LSC is expected to maintain a clean, safe, and healthy environment for the individuals. Qualifications: High School Diploma and 2 years of related experience with adults with intellectual disabilities Must be 21 years old or older Certification in CPR required prior to hire Valid CT Driver’s license, clean driving record, and ability to obtain and maintain approval from leased vehicle company Must obtain and maintain all certifications/licenses such as Physical Management Techniques (PMT), DDS Medication Certification, etc. as required by DDS standards Essential Duties and Responsibilities: Ensure a safe and accessible environment for all individuals Implement all safety protocols and adhere to agency and state guidelines, policies, and procedures Exhibit a caring, empathetic attitude towards the individuals while remaining sensitive to their needs Use incidental teaching to model appropriate behavior/activities for individuals during instructions, enhancing their ability to complete tasks independently Provide transportation for individuals when needed, including driving with agency or personal vehicle as necessary Handle personal hygiene of assigned individuals (bathing, showering, wiping, dressing, cleaning, and other necessary tasks) Adhere to dietary guidelines and food consistency instructions for individuals Ensure appropriate accountability supervision to maintain health and safety as outlined in policies and individuals’ person-centered plans Implement strategies from Behavioral Support Plans and/or Behavioral Guidelines for individuals Advocate for individuals, helping them make choices in their environment and ensuring their wishes, values, and beliefs are considered in decisions Track and maintain current required training/certifications, ensuring recertifications are updated before expiration dates Accurately implement and complete required programming and documentation (e.g., Electronic Visit Verification System, therapy documentation, BER, GER, T-logs, program documentation, data collection, financial records, scheduled activities) Establish and maintain professional and positive relationships with community contacts to foster inclusion and facilitate participation Log hours worked using the ADP biometric time clock system Perform all assignments as outlined in the Life Skills Coach handbook and as assigned by supervisor Other duties as assigned Reports to: DSH Supervisor #J-18808-Ljbffr W H Community House
