HEALTH SERVICES REPRESENTATIVE - 64057641 Requisition No: 875110 Agency: Department of Health Working Title: HEALTH SERVICES REPRESENTATIVE - 64057641 The Linkage to Care Coordinator (LTC)/Disease Intervention Specialist (DIS) representative is a regional position within Area 15 (St Lucie, Martin, Okeechobee, Indian River County). The incumbent identifies, links, and re‑engages newly diagnosed and previously diagnosed HIV‑positive individuals into the HIV/AIDS system of care. Responsibilities include managing non‑compliant clients, conducting community and clinic counseling and testing, ensuring linkage within 30 days, maintaining records in the HIV/AIDS linkage module, and providing timely follow‑up. The role requires phlebotomy training, Disease Intervention Specialist certification, and the ability to travel out of county and work before, after hours, and on weekends. Job Responsibilities • Identify newly diagnosed and re‑engaged clients and assist providers in locating non‑compliant clients. • Ensure all newly diagnosed individuals are linked to care within 30 days and re‑engaged within required timelines. • Coordinate with local Regional Linkage to Care Coordinator (RLC), HIV/AIDS surveillance, and STD program staff to complete data‑to‑care processes and completed required forms. • Identify barriers to care and provide referrals and practical support such as transportation, medication assistance, and insurance navigation. • Consult with public or private personnel and health care providers to obtain necessary information and secure permission to apply epidemiologic processes to private patients. • Maintain confidentiality, complete documentation in the Health Management System (HMS), CAREWare, STARS, and other reporting systems. • Perform case management activities, including HIV/STD testing and counseling, Ryan White eligibility, and Test and Treat linkage. • Provide partner services interviews and offer HIV/STD testing to at‑risk individuals during outreach and community events. • Serve as back‑up to the Jail Linkage Coordinator for HIV/STD counseling, testing, data collection, and education activities. • Maintain client confidentiality, submit required logs, reports, and attend departmental meetings. Required Knowledge, Skills, and Abilities • Willingness to travel in and out of Area 15 and between counties. • Valid driver’s license and ability to lift/move up to 50 pounds. • Case manager experience, phlebotomy training, and self‑motivated initiative. • Ability to create and deliver PowerPoint presentations and perform effective communication with diverse communities. • Proficiency with Microsoft Office (Word, PowerPoint, Excel, Outlook). Qualifications Minimum Must be willing to become certified in phlebotomy and complete Disease Intervention Specialist and Linkage to Care ARTAS training. Must be willing to travel out of county, including undesirable neighborhoods. Must be willing to work before and after hours, and during weekends. Must be willing to serve in special needs shelters during hurricanes and/or other emergency duties. Must have at least 1‑year case management experience and/or patient care coordination navigation experience. Must have a valid driver’s license. Must pass an additional required background screening conducted. Must be able to lift/move up to 50 pounds, bend, stoop, reach, and climb a ladder. Preferred Certified or trained phlebotomist. Experience working with HIV/STD programs and related work. Experience performing file reviews and reports. Experience working with detention facilities, drug rehabilitation facilities, mental health facilities, or related work. Proficiency with Microsoft Word, Excel, PowerPoint, and other programs. Case management experience or experience with caseloads. Experience of providing risk reduction education on HIV, STDs, safe sex practices, and/or conducting community events. Experience working with marginalized, low‑income, and disenfranchised communities. Bilingual (Spanish and/or Creole) preferred.
