Seeking a licensed Physical Therapist to provide comprehensive therapy services for older adult participants in a community-based care program. Key Responsibilities Include Performing assessments and developing personalized treatment plans in collaboration with an interdisciplinary team. Administering therapy treatments to improve mobility, strength, and independence. Evaluating and recommending adaptive equipment, home modifications, and safety measures. Educating participants and families on therapy procedures, home programs, and safety techniques. Training staff on proper body mechanics, equipment use, and transfer techniques to ensure safety. Documenting assessments, plans, treatments, and progress accurately and timely. Participating in team meetings, quality improvement initiatives, and community events. Maintaining professional licensure, certifications, and continuing education requirements. Qualifications And Skills Advanced degree in Physical Therapy from an accredited institution. Valid license to practice Physical Therapy in the state. Experience working with elderly or frail populations preferred. Bilingual skills in Hmong are a plus. Valid driver’s license and auto insurance. Ability to collaborate effectively within an interdisciplinary team.
