Start Date: ASAP Job Requirements Active Registered Nurse license in the appropriate state Minimum 2 years of bedside nursing experience ICU experience or strong clinical skills appropriate for an LTACH setting BLS certification required before first shift ACLS certification required before first shift Strong assessment, documentation, and communication skills Ability to work independently while supporting a collaborative care team Responsibilities Perform nursing assessments within 8 hours of admission Develop and follow individualized plans of care for LTAC patients Identify teaching needs and patient/family goals based on assessment findings Provide timely updates for initial and weekly interdisciplinary team conferences Document assessments, interventions, and patient responses accurately and promptly Participate in the patient rehabilitation program and support direct care needs Initiate discharge planning and patient education early in the admission process Communicate changes in patient status to physicians, charge nurses, and care team members Delegate and supervise ancillary nursing staff appropriately Promote patient, staff, and environmental safety at all times Support unit teamwork, supply awareness, and efficient use of resources Participate in professional growth and ongoing learning opportunities Why Apply?
