Conduct outreach to people who use drugs, people experiencinghomelessness, and communities throughout Will County to educate onoverdose prevention and distribute overdose prevention supplies. Develop and direct prospective referrals to FGC and othertreatment providers, as appropriate and in accordance with patients’needs and wishes. Develop referral relationships with shelter partners, homelessoutreach teams, local task forces, hospital social workers,faith-based and local community organizations, and other communitybased organizations as appropriate, serving as their liaison toaccessing treatment services offered by FGC Demonstrate professionalism in working with patients, staff andother outside professionals or collaterals. Participate in supervision with Program Manager as well asappropriate continuing education activities related to caremanagement and maintenance of relevant certification or licensure Describe FGC programs, services and eligibility requirements inlaymen’s terms to people struggling with opioid and alcoholaddiction and those at risk of overdosing, as well as family membersor friends of people struggling with addiction, informing andassisting them to gain immediate access to treatment Work Schedule: Monday - Friday (7:30a - 4:00p) Start Date: ASAP Work Location: Joliet, IL & Aurora, IL Starting Pay Rate: $20.00 - $21.00 per hour Family Guidance Centers, Inc. is a Drug Free Environment(including THC and/or CBD).
