Licensed Practical NurseMajor duties include, but are not limited to:Processes Community Care-GEC Nonskilled Homemaker Consults using the Health Share Referral Manager (HSRM), CPRS, and other recognized electronic software to schedule, generate referrals and create authorizations for Veterans receiving Nonskilled care in the community.The LPN must create authorizations accurately and timely following Office of Community Care procedures and guidelines.Actively participates in vendor management: Listens to concerns and communicates effectively.Avoids any vendor relationship or activity that might impair or appear to impair the ability to make objective and fair decisions when processing consults, creating authorizations, and in the performance of the job.Performs electronic or telephonic contacts to complete the documentation required for initiating referrals/authorizations processed.Follows up with community vendors to ensure appointment completion and obtaining medical records as required.Obtains medical documentation from community providers, scans to appropriate consults within the Computerized Patient Record System (CPRS), and alerts VA providers for continuity of care. Coordinates internal and external care between customers and providers.Appropriately documents all communications with Veterans, family caregivers, community, and internal stakeholders in CPRS.Assists in collecting performance improvement data for skilled and unskilled care, including patient satisfaction surveys.Performs duties as a team member, willingly accepts all assignments, and demonstrates flexibility and competence to carry out tasks.Promotes a climate conducive to high morals and recognition of individual worth.Ability to independently set priorities and organize work to meet deadlines, ensuring compliance with established processes, policies, and regulations.Communicates any clinical, legal, or ethical concerns with supervisors.Work Schedule: Monday-Friday - 07:30am-4:00pm. Work schedule is subject to change based on Agency needs.Pay: Competitive salary and regular salary increases.Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year).
