What You'll Do As an LPN at Carillon Oaks at Lincoln, you will play an important role in supporting the health, safety, and well-being of our residents. Key responsibilities include:
- Provide compassionate nursing care in accordance with residents' individualized care plans
- Administer medications and treatments as ordered by physicians and in accordance with applicable regulations
- Monitor residents for changes in condition and promptly communicate concerns to the appropriate clinical leadership
- Perform routine nursing assessments and document resident care accurately and timely
- Coordinate with physicians, pharmacies, families, and other healthcare providers as needed
- Respond appropriately to resident needs and changes in condition
- Provide guidance and support to resident care staff
- Assist with admissions, assessments, and ongoing resident care planning
- Maintain accurate clinical documentation and medication records
- Promote resident dignity, independence, choice, and quality of life
- Communicate professionally and compassionately with residents and their families
- Collaborate with leadership and fellow team members to ensure continuity and quality of care
- Follow all applicable state regulations, company policies, and infection-control practices
- Help maintain a positive, respectful, and team-oriented environment throughout the community
- Current and unrestricted Licensed Practical Nurse (LPN) license
- Previous assisted living, senior living, skilled nursing, or long-term care experience preferred
- Strong clinical judgment and attention to detail
- Excellent communication and interpersonal skills
- Ability to effectively prioritize resident needs in a fast-paced environment
- Dependable, organized, and committed to providing high-quality care
- Strong teamwork and problem-solving skills
- A genuine passion for working with seniors and supporting their quality of life
