Bilingual Registered Nurse Case Manager

Your future role at a glance Location: Austin, TX Department/Specialty: Mission Integration Schedule: Monday - Friday; 8A-5P CST Salary: $84,061 - $118,668 per year #LI-Remote How you'll make an impact in this role Coordinate the overall interdisciplinary plan of care for patient, from admission to discharge. Create plan for care across the continuum, integrating patient/family preferences and values. Monitor patient care through assessments, evaluations and/or patient records. Advocate for resources and removal of barriers. Maintain ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan. Act as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care. What minimum qualifications you'll need Licensure / Certification / Registration: Registered Nurse obtained prior to hire date or job transfer date required. Licensure required relevant to state in which work is performed. Case Manager credentialed from the American Case Management Association (ACMA) preferred. BLS Provider preferred. American Heart Association or American Red Cross accepted. Case Manager credentialed from the Commission for Case Manager Certification (CCMC) preferred. Licensure required relevant to state in which work is performed Education: Diploma from an accredited school/college of nursing OR Required professional licensure at time of hire.
