We are seeking a compassionate and motivated Physical Therapist to join our rehabilitation team. In this role, you will evaluate patients, develop individualized treatment plans, and provide high-quality therapy services to help patients improve mobility, restore function, manage pain, and regain independence.

Responsibilities

• Evaluate patients and perform comprehensive physical therapy assessments.
• Develop and implement individualized treatment plans based on each patient's diagnosis, needs, and goals.
• Provide therapeutic interventions to improve strength, mobility, balance, coordination, endurance, and functional independence.
• Educate patients and family members on treatment plans, exercises, safety, injury prevention, and ongoing care.
• Monitor patient progress and modify treatment plans as appropriate.
• Maintain accurate and timely clinical documentation in accordance with organizational and regulatory requirements.
• Collaborate with physicians, nurses, occupational therapists, speech therapists, and other members of the healthcare team.
• Assist patients with mobility, transfers, gait training, balance, and functional activities.
• Ensure a safe and supportive environment for patients throughout the rehabilitation process.
• Participate in interdisciplinary team meetings and discharge planning when appropriate.
• Maintain compliance with all applicable professional, state, federal, and organizational regulations.

Qualifications

• Graduate of an accredited Physical Therapy program. DPT
• Current Physical Therapist license in the applicable state.
• Strong clinical assessment and treatment skills.
• Excellent communication and interpersonal skills.
• Ability to work effectively as part of an interdisciplinary team.
• Strong organizational skills and attention to detail.
• Commitment to providing compassionate, patient-centered care.

Preferred Qualifications

• Previous experience in a rehabilitation setting is preferred.
• Experience working with patients recovering from orthopedic injuries, neurological conditions, surgery, illness, or other conditions affecting mobility and function.
• Experience in inpatient rehabilitation, skilled nursing, outpatient rehabilitation, or acute care is a plus.
