Hybrid RN Pathways Facilitator - Telephonic Hospice Care

Empath Health in Florida is seeking a hybrid role specialist to provide telephonic patient support and on-site visits, with a schedule of Monday–Friday, 8:30 AM–5:30 PM at the St. Petersburg office. The position focuses on coordinating hospice care, education for families, and advocacy within the Pathways program. Two years of hospice experience are required along with an AA degree or equivalent.
