DEPARTMENT: AdministrationSUPERVISOR: Administrator and/or Corporate Designee POSITION SUMMARY:The Social Service Director is responsible for the provision of resident services to improve and/or maintain their psychosocial well-being.• The Social Service Director frequently rounds the building to visit with the residents to assess psychosocial needs. SSD is responsible for reviewing 24- hour nursing report to see if documentation suggests need for psychosocial intervention.• The Social Services Director will maintain a list of residents to see psychosocial clinicians upon their next visit- medical, psychological, and social workers.• The Social Services Director may seek resources for resident intervention from outside providers when needed.• The Social Services Director maintains positive work relationships with case managers and providers.Along with the other duties listed in the job description. QUALIFICATIONS/ EXPERIENCE/ REQUIREMENTS:Minimum QualificationsFreedom from illegal use of drugs.Freedom from use and effects of drugs and alcohol in the workplace. Anyone found guilty by a court of law of abusing, neglecting, or mistreating individuals in a health care related setting are ineligible for employment in the position.Education and/or ExperienceAn individual with a minimum of a bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology; and one year of supervised social work experience in a health care setting working directly with individuals. Experience in Long-Term care setting preferred but not required.
