Position Highlights

• Coordinate patient-centered care: Facilitate collaboration among patients, families, and healthcare teams to develop and implement individualized care plans, ensuring high-quality outcomes and improved patient experiences.
• Manage transitions and discharge planning: Conduct assessments, coordinate care resources, and oversee safe, efficient hospital transitions while maintaining continuity of care and supporting medical management initiatives.
• Ensure compliance and optimize reimbursement: Evaluate admissions and ongoing stays using approved criteria, communicate with payers to maximize reimbursement, and adhere to all compliance regulations and hospital policies.

• Non-bedside employee
• Full-time, 32 hours per week
• 1st shift position
• Weekend shift rotation

Qualifications:

• Successfully passed Board of Registration in Nursing Examination and is licensed in the Commonwealth of Massachusetts
• Current American Heart Association Health Care Provider CPR certification required.
• Education: Diploma in Nursing
