• Conduct in-depth assessments of patients and families to evaluate their psychosocial needs and create individualized care plans that address both emotional and practical aspects of living with a terminal illness.

• Provide therapeutic interventions and counseling in both individual and group settings, utilizing evidence-based practices that cater to unique patient and family circumstances.

• Maintain meticulous and confidential records of patient interactions and treatment plans using our advanced electronic health record (EHR) system to ensure compliance with healthcare regulations.

• Collaborate closely with physicians, nurses, and other healthcare professionals to create a holistic care approach that prioritizes patient comfort and family involvement throughout the end-of-life process.

• Stay current with advancements in palliative care and social work practices through continuous education and professional development opportunities.

This position primarily focuses on patients facing life-limiting conditions, providing them and their families with the support needed to navigate the complexities of end-of-life care. You will address a range of emotional challenges, including grief, anxiety, and depression, ensuring that each patient’s unique cultural and personal values are respected and integrated into their care plan.

• A Master's Degree in Social Work from an accredited institution is mandatory.
\n
• An active LICSW license in the state of Vermont is required for this position.
\n
• Strong communication skills and the ability to empathize with patients and families during difficult circumstances are essential to success in this role.

• Experience working in hospice or palliative care settings is preferable but not required.
\n
• Proficiency in using electronic health record systems and familiarity with evidence-based therapeutic methodologies are advantages.
