Veterans Outpatient RN Care Manager

STGi is seeking a Registered Nurse Care Manager to provide clinic-based services at our Community Based Outpatient Clinic in Savannah, TN. The RN will advocate for Veteran patients, coordinate care with the VA Medical Center and stakeholders, and manage patient care under standard clinic protocols. The role emphasizes patient flow, triage, education, and collaboration with PCPs to ensure quality, uninterrupted care while maintaining safety and confidentiality in a fast-paced outpatient setting. #J-18808-Ljbffr STGi
