Case Manager Registered Nurse - Field - Must reside in Lake County Illinois

Position Summary This Case Manager Registered Nurse position is with Aetna’s Long-Term Services and Supports (LTSS) team and is a field-based position out of Lake County IL (Applicable Zip Codes: 60031, 60064, 60087, 60085, or 60099 ). The requirements is for candidates to hold an Illinois Registered Nurse License, and travel up to 75% of the time to meet with members face to face. The schedule for this position is Monday-Friday 8:00 am - 5:00 pm CST. Coordinating all case management activities with members to evaluate the medical needs of the member to facilitate the member’s overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member’s overall wellness through integration. Services strategies policies and programs are comprised of network management and clinical coverage policies. This is an Exempt position. Required Qualifications Minimum 2+ years of clinical experience required Active and unrestricted RN license in the state of IL Must reside in Lake County IL (Applicable Zip Codes: 60031, 60064, 60087, 60085, or 60099). Must possess reliable transportation and be willing and able to travel up to 50-75% of the time to meet with members face to face in the Lake County IL area. Mileage is reimbursed per our company expense reimbursement policy Preferred Qualifications Case management and discharge planning experience Managed Care experience Microsoft Office Suites experience Education Minimum of Associate’s Degree in Nursing required Bachelor’s Degree in Science and Nursing preferred Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $66,575.00 - $142,576.00 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above. Our people fuel our future.
