Licensed Social Worker Full-Time | Park Manor Health &rehabilitation | Northport, AL Compensation: Competitive Pay Based on Experience and Qualifications Make a Difference Beyond the Clinical Care Park Manor Health &rehabilitation is seeking a compassionate and organized Licensed Social Worker to join our interdisciplinary care team. The Social Worker plays an important role in supporting residents and families throughout their stay, helping coordinate services, address psychosocial needs, and plan for safe and successful transitions of care. Responsibilities Assess residents' psychosocial needs and develop appropriate care plans. Provide support and resources to residents and families. Participate in care plan meetings and interdisciplinary team collaboration. Assist with discharge planning and coordinate community services and resources. Support residents with adjustment, emotional, social, and family concerns. Maintain accurate and timely documentation. Assist with resident rights, grievances, advance directives, and regulatory requirements. Qualifications Current Social Work license in the State of Alabama – REQUIRED. Degree in Social Work from an accredited program. Skilled nursing, rehabilitation, long-term care, hospital, or healthcare social work experience preferred. Knowledge of discharge planning and community resources preferred. Strong communication, organization, problem-solving, and interpersonal skills.
