Key Responsibilities of the Physical Therapist

• Conduct home health therapy services with adult patients
• Manage chronic diseases, wound care, and advanced clinical procedures including IV, pleurx, and wound vac
• Utilize Homecare Homebase EMR system for accurate charting
• Provide disease education and create maintenance notes for geriatric/chronic disease management

Requirements of the Physical Therapist

• Bachelor's Degree in Physical Therapy
• Minimum of 1 year of experience in home health settings
• Active State Licensure and BLS certification
• Driver's License and proof of insurance
• Proficiency in Homecare Homebase EMR and OASIS-E assessment tool
