Gathers information for both new and established patients such as medical history, diagnostic tests, comprehensive physical examination, behavioral, development and psychosocial assessments. 2. Assesses and identifies patient/family physical, psychosocial, educational, and cultural needs for assigned patient population. 3. Accurately interprets health assessment data and applies findings in directing patient care. 4. Provides ongoing and follow-up care including appropriate pharmacologic and diagnostic treatments and patient/family education. 5. Documents all health data including plan of care and care according to policy, patient/family outcomes, and reassessments and plan modifications. 6. Accurately completes all billing forms, including diagnostic (ICD-9) and treatment (CPT) codes, based on federal and state laws and AC policy. 7. Communicates with referring physicians, health providers, and hospital staff as appropriate. 8. Actively participates in quality improvement activities and the development strategies to improve patient care delivery and cost effectiveness. 9. Disseminates relevant research findings through practice, publication, education, and/or consultation. 10. Participates in AC, local and national professional nursing organizations as appropriate to maintain and share expertise. 11. Performs other duties as assigned.
