Home Health Occupational Therapist

CenterWell Home Health seeks a Home Health Occupational Therapist to deliver personalized, in-home therapy that maximizes patient independence, safety, and quality of life. In this field-based role, you will evaluate functional abilities, design and implement evidence-based treatment plans, and provide ADL training, home safety assessments, and caregiver education. You’ll collaborate closely with an interdisciplinary team, document care in EMR, and support CenterWell’s culture of compassion, integrity, and continuous improvement while growing professionally in a supportive environment.
