We are seeking a Linkage to Care Specialist to help individuals living with infectious diseases access and remain connected to medical care, supportive services, and available resources. This position combines client navigation, care coordination, engagement, data collection, and quality improvement to help improve health outcomes and strengthen service delivery throughout the Charlotte-Metro area.

The ideal candidate is compassionate, organized, resourceful, and comfortable working with individuals facing complex social and socioeconomic barriers to healthcare. This role also requires strong analytical and communication skills and the ability to collaborate with healthcare providers, case managers, community organizations, and regional partners.

What You’ll Do

Linkage to Care & Client Navigation

• Connect individuals who are newly diagnosed with infectious diseases to appropriate medical care and supportive services.
• Reengage individuals living with infectious diseases who have been lost to care and help them return to appropriate medical services.
• Identify and assess barriers that may prevent clients from accessing or remaining engaged in care, including socioeconomic and other communitybased barriers.
• Develop and implement individualized care and linkage plans based on client needs.
• Help clients navigate medical care, social support, mental health services, and other resources.
• Maintain accurate client records and collect required service and outcome data.
• Develop and maintain protocols for identifying newly diagnosed individuals and those who have fallen out of care.
• Conduct outreach and followup to locate individuals who are lost to care and determine whether and where they are receiving services.
• Develop and maintain relationships with outside partners to strengthen referral and linkage networks.
• Promote and coordinate intervention activities with local partners.
• Prepare and submit monthly service reports and quarterly program narratives.
Quality Improvement & Performance
• Conduct quality assurance and quality improvement activities related to infectious disease services.
• Collect, monitor, analyze, and report quality improvement performance indicators and measures.
• Use evaluation and performance data to identify opportunities to improve services, processes, and client outcomes.
• Assist with developing protocols, measurement tools, and data collection processes.
• Analyze statistical and program data and apply findings to improve administrative and service systems.
• Participate actively in the Regional Quality Council (RQC) and related national quality improvement initiatives.
• Collaborate with Bridge Counselors, RQC members, ID clinics, and other multidisciplinary partners to identify and address barriers to care.
• Participate in quality management meetings and regional initiatives focused on improving care engagement and outcomes.
• Collaborate with healthcare and other partners to identify individuals who remain out of care and develop strategies to reconnect them with appropriate services.
Professional Engagement & Outreach
• Build and maintain effective relationships with healthcare providers, local organizations, social service agencies, and other stakeholders.
• Represent the organization professionally in public settings and partner meetings.
• Attend conferences, educational webinars, trainings, and professional meetings to remain current on care, linkagetocare strategies, quality improvement, and evidencebased best practices.
• Perform other duties and responsibilities as assigned.
What We’re Looking For The successful candidate will demonstrate:
• Experience working in healthcare, case management, care coordination, public health, infectious disease services, social services, or a related field.
• Strong client navigation, advocacy, and problemsolving skills.
• Ability to work effectively with individuals experiencing complex or difficult circumstances.
• Strong organizational skills, including the ability to manage multiple priorities, deadlines, projects, and client needs.
• Excellent written and verbal communication skills.
• Ability to collaborate effectively with multidisciplinary teams and local partners.
• Strong criticalthinking and solutionoriented problemsolving abilities.
• Ability to collect, organize, interpret, and report program and quality improvement data.
• Strong computer skills, including proficiency with Microsoft Office and the ability to learn and use electronic databases and other information systems.
• Ability to work independently, exercise sound judgment, and take initiative.
• Flexibility and adaptability.
• Strong presentation and publicspeaking skills.
• Professionalism and discretion when working with confidential client information.
