This role collaborates closely with physicians, nursing staff, rehabilitation professionals, patients, and families to develop effective treatment plans and achieve optimal outcomes.Occupational Therapist ResponsibilitiesEvaluate patients within required regulatory timeframes following physician referralDevelop individualized treatment plans and establish measurable goalsProvide occupational therapy services in accordance with physician orders and professional standardsAssess patients' abilities to perform activities of daily living (ADLs)Supervise Occupational Therapy Assistants (COTAs), aides, and students as appropriateCollaborate with physicians, nursing staff, and interdisciplinary team members regarding patient progress and care plansParticipate in care conferences, rehab meetings, and discharge planningEducate patients, families, caregivers, and staff on treatment plans and therapy techniquesRecommend adaptive equipment, assistive devices, and durable medical equipment as neededComplete accurate and timely documentation in compliance with federal, state, and facility guidelinesMaintain therapy equipment and report maintenance or repair needsPromote a positive, compassionate, and patient-focused rehabilitation environmentQualificationsCurrent and active California Occupational Therapist License requiredBachelor's degree in Occupational Therapy requiredMaster's or Doctorate in Occupational Therapy preferredSkilled nursing, rehabilitation, post-acute, or long-term care experience preferredStrong clinical assessment and treatment planning skillsExcellent communication and interpersonal abilitiesExperience with PointClickCare and Casamba preferredAbility to work effectively in a fast-paced, team-oriented environmentCommitment to delivering exceptional patient-centered careAbout Citrus Heights Post AcuteAt Citrus Heights Post Acute, we are committed to helping our patients achieve their highest level of independence and recovery.
