Youth Transitional Case Manager — Impact & Licensure Support

Thresholds, Inc. seeks a Case Manager for the Youth Transitional Living Program in Chicago. You will coordinate treatment, develop individualized service plans, and connect youth ages 17–24 with school, medical providers, family and community resources. You will conduct assessments, provide case management and group supports in the community and at Thresholds locations, and facilitate transitions with a focus on safety and independence. #J-18808-Ljbffr Thresholds-Inc
