Hope Creek Nursing and Rehabilitation Center, a leading provider of healthcare services in East Moline, Illinois, is seeking a compassionate and skilled Physical Therapist (PT) to join our dynamic team!

About the Role:

As a Physical Therapist at Hope Creek Nursing and Rehabilitation Center, you will play a vital role in helping our residents achieve their optimal level of physical function and well-being. You will work closely with our interdisciplinary team to develop and implement comprehensive rehabilitation plans, focusing on patient-centered care and outcomes-driven treatment. Your expertise and passion for physical therapy will enable our residents to regain strength, mobility, and independence, empowering them to live their best lives.

Responsibilities:

• Conduct thorough initial and ongoing assessments to identify patient needs and develop individualized rehabilitation plans

• Implement evidence-based physical therapy interventions, including exercise programs, manual therapy, and modalities

• Collaborate with our healthcare team to develop and update patient treatment plans, ensuring seamless transitions and optimal outcomes

• Conduct patient education and training on proper body mechanics, safety, and self-care techniques

• Maintain accurate and detailed records of patient progress and treatment plans

• Participate in quality improvement initiatives to drive continuous improvement and excellence in patient care

Requirements:

• Must be a licensed Physical Therapist (PT) with a degree from an accredited Physical Therapy program

• Must have a strong foundation in physical therapy principles, techniques, and best practices

• Must be able to work effectively in a fast-paced, dynamic environment with a focus on patient-centered care

• Must be able to lift, push, and pull equipment and supplies as needed
