Schedule: M-F, Days; Rotating weekend coverage
Assignment Details: Guaranteed Hours: 40 Contract Length: 13 weeks
Requirements:
- Current Professional Licensure Required
- Current IL Physical Therapist License Required
- Valid Driver's license and proof of personal auto insurance Required
- BLS Required
- APTA Membership Preferred
- Physical Therapy minimum 1 year Preferred
- Strong computer skills including Microsoft Word, Excel and Outlook
Roles & Responsibilities:
- Evaluate, plan, direct, and administer Physical Therapy services as prescribed by a licensed Provider.
- Treat patients using Physical Therapy modalities and skilled therapy techniques; instruct patients on the use of adaptive/assistive equipment.
- Supervise and direct Physical Therapy Assistants; maintain open communication with members of the Rehabilitation team.
- Perform appropriate assessments on all patients; interpret results within scope of licensure.
- Design treatment programs for patients with various disabilities and medical problems.
- Formulate realistic short and long-term treatment goals in collaboration with the patient and family.
- Ensure accurate, timely documentation and compliance with regulatory standards.
Compliance / Notes:
- Comply with all established safety procedures to ensure a safe environment for patients, visitors, and staff.
- Participate in performance improvement activities.
