Provider Type: Physician Coverage Type: Scheduled Clinical Hours Plus Call EMR System: Epic Practice Setting: Outpatient Patients Per Shift: 12-15 Does your Facility require physicians to be board certified?: Yes Fellowship: NA Minimum Board Certification: Certified Estimated Credentialing Timeframe (Days): 90 What is the license requirement to bid?: None Monday- Friday clinic only (8am-5pm) 1 week a month is needed. HIGHLY prefer one provider to cover the coverage need but will look at availability. Provider will see anywhere from 12-15 patients a day. We are flexible so if the provider is only available for 4 days, we would consider. No privileges required so they can start as soon as set up. We have 04/14 as a start here but it could be later depending on how fast a candidate is sourced.
