Speech-Language Pathologist (SLP) – Per Diem / PRN Home Health

Pay: Competitive per-visit compensation ($80.00+ per visit )

Location: Oahu, Hawaii (reliable transportation required – we serve patients throughout the island)

Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload

We are seeking a compassionate and motivated Speech-Language Pathologist (SLP) to provide skilled in-home speech therapy services to adult and geriatric patients throughout Oahu.

Responsibilities

• Provide skilled home health speech therapy services in patients' homes

• Complete speech-language pathology evaluations and assessments

• Develop and implement individualized treatment plans

• Treat swallowing disorders ( dysphagia ), cognitive-communication deficits, speech and language impairments, and voice disorders

• Educate patients, caregivers, and family members

• Collaborate with nurses, physical therapists, occupational therapists, and case managers

• Maintain accurate documentation in accordance with Medicare home health regulations

Qualifications

• Current Hawaii license as a Speech-Language Pathologist (SLP)

• Certificate of Clinical Competence ( CCC-SLP ) preferred

• 1+ year of clinical experience (home health experience preferred)

• Knowledge of Medicare home health documentation requirements preferred

• Strong communication and organizational skills

• Ability to work independently in the field

• Valid driver's license and reliable transportation
