RN Case Manager

Job Summary:

We seek a highly skilled and experienced Registered Nurse specializing in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room to serve as a Case Manager for Purchased Referred Care (PRC) at Fort Peck Service Unit. The RN will provide skilled nursing care, case management, and coordinate care for patients requiring advanced medical services. The role involves patient assessments, medication administration, treatment planning, and collaboration with interdisciplinary teams to ensure quality care. Strong communication and collaboration skills are essential. A minimum of two years of relevant experience and active RN licensure are required. Responsibilities:

• Provide skilled nursing care and case management for patients under Purchased Referred Care programs.
• Conduct patient assessments and develop individualized treatment plans.
• Administer medications and monitor patient responses.
• Collaborate with interdisciplinary healthcare teams to coordinate patient care.
• Manage documentation and reporting related to PRC case management.
• Communicate effectively with patients, families, and healthcare providers.
• Ensure compliance with healthcare policies and procedures.

Required Skills & Certifications:

• Active Registered Nurse license in the state of employment.
• Minimum of two years of experience in hospital, home health, or hospice care.
• Clinical assessment, medication administration, and treatment planning skills.
• Interdisciplinary team collaboration experience.
• Excellent communication and interpersonal skills.
• Bachelor's degree in Nursing or related field.
• Driver's license, car insurance, and reliable automobile for field work.
• Experience with Indian Health Service Purchased Referred Care (PRC) preferred.

Preferred Skills & Certifications:

• Experience in Medical/Surgical, ICU, ER, Psychiatric, Labor & Delivery, PACU, or Operating Room specialties.
• Prior case management or care coordination experience.
• Familiarity with tribal health systems and protocols.

Special Considerations:

• Position is for Fort Peck Service Unit with a contract duration from September 21, 2026, to March 27, 2027.
• Must be available to start as soon as cleared.
• Field work requiring travel and use of personal vehicle may be necessary.

Scheduling:

• Work schedule and shift assignments to be determined based on service unit needs.
• May involve day shifts with possible flexibility depending on patient care requirements.
