Job Description & Requirements

• Provide direct patient care for medically complex, long-stay patients
• Monitor vital signs and patient status , identifying changes and intervening as needed
• Administer medications and IV therapies (including antibiotics, TPN, drips)
• Perform wound care, trach care, and ventilator management
• Develop, implement, and update individualized care plans [interviewguy.com]
• Collaborate with physicians, respiratory therapy, PT/OT, and case management
• Educate patients and families on conditions, recovery, and discharge planning
• Accurately document care, assessments, and outcomes
