RN – MDS Coordinator

Location: Platte, South Dakota
Job Type: Contract (13 Weeks)
Pay Rate: $45.00 – $52.50/hour (Based on Experience)
Shift: Days
Schedule: 8:00 AM – 4:30 PM
Guaranteed Hours: 36 Hours/Week

We are seeking an experienced Registered Nurse (RN) – MDS Coordinator to support resident assessments, care planning, and reimbursement documentation within a long-term care setting. This role offers a consistent daytime schedule and the opportunity to play a key role in resident care quality and regulatory compliance.

• Active RN License.

• Previous MDS Coordinator experience preferred.

• Experience working in Long-Term Care or Skilled Nursing settings.

• Strong assessment, care planning, and documentation skills.

• Knowledge of Medicare, Medicaid, and regulatory compliance requirements.

• Experience with electronic medical records; EPIC experience preferred.

• Ability to collaborate effectively with interdisciplinary teams.

• Complete and coordinate MDS assessments accurately and within required timelines.

• Ensure compliance with Medicare, Medicaid, state, and federal regulations.

• Develop, implement, and update individualized resident care plans.

• Collaborate with nursing staff, therapy teams, physicians, and other healthcare professionals.

• Monitor documentation accuracy to support reimbursement and quality measures.

• Assist with quality improvement initiatives and survey preparedness activities.

• Maintain accurate resident records and assessment documentation.
