Schedule: Monday through Friday 8 am to 5 pm. On call one weekend a month 8am to 8am. On call 2 weeknights a month 5 pm to 8am. Work two holidays a year on a rotation.

Territory: Macedonia, Twinsburg, Aurora, Hiram, Hudson, Kent, Streetsboro, Ravenna, Rootstown, Cuyahoga Falls, Stow areas

Guaranteed base hourly rate + uncapped bonus opportunities. Mileage reimbursement. Provide meaningful care beyond the bedside.

As a Home Health LPN, you'll provide one-on-one skilled nursing care, build meaningful relationships with patients and families, and work alongside a supportive interdisciplinary team focused on positive patient outcomes.

What You'll Do

• Provide skilled nursing care in patients' homes
• Follow physician orders and care plans created by the Case Manager
• Monitor patient conditions and report changes
• Administer medications and treatments as prescribed
• Educate patients and families using teach-back methods
• Help reduce hospital readmissions through patient education and preventative care
• Complete clinical documentation in accordance with agency and Medicare guidelines
• Communicate schedule or availability changes with the appropriate team members

Qualifications

• Current Licensed Practical Nurse (LPN) license
• Minimum of 1 year of LPN experience in an acute care setting
• Home health experience preferred
• Valid driver's license and active auto insurance
• Strong communication, time management, and documentation skills
• Compassionate, dependable, and patient-focused
