Job Description Beth Israel Lahey Health Primary Care (BILHPC) is building a high‑performance primary care model that will make the organization a leader in patient experience, provider and care team work life, and quality of care. We are also establishing a diabetes education program to provide quality care and medication‑assisted therapy for patients with diabetes in an outpatient setting. Primary Responsibilities Perform all duties of a Registered Dietitian, including coordinating and facilitating quality clinical management of a broad‐based outpatient population. Collaborate with providers to identify needs for preventive care, evidence‑based chronic disease management, and care coordination; provide general and specific health education. Work directly with patients to identify and monitor self‑management goals. Collaborate with community‑based providers; follow up on ED visits and inpatient discharges; assist providers with exams, treatments and procedures as needed. Provide skilled telephonic triage and assessment, education, collaboration and coordination of healthcare and community resources. Accurately document all aspects of patient care in the Electronic Health Record in a timely basis; schedule tests and referrals for specialty care on provider order as needed. Monitor and ensure follow‑up of all abnormal test and imaging results under provider direction. Oversee inventory and ordering management of clinical supplies and equipment as necessary. Provide oversight, delegation and act as a resource to unlicensed staff; provide ongoing education and training to practice staff. Update clinical knowledge and skills through consultation, literature review, and formal and informal education including CEUs; maintain compliance with mandatory annual training. Ensure PHI is used only for treatment, payment and operations; promote patient safety practices; report adverse events and near misses to BIDHC Risk Management. Perform specific diabetes education duties: conduct clinical and psychosocial assessment of diabetes patients in individual or group settings; identify educational needs; develop and implement individualized plans of care; empower patients with knowledge and skills for optimal control; educate on medication administration, SMBG, CGMS, pump therapy, nutrition, exercise, recognition and treatment of acute complications. Monitor patients’ response to therapy and revise plans as indicated; prevent/decrease incidence of complications by supporting preventive health risk behaviors. Conduct ongoing quality improvement studies; support hospital policies and procedures while sustaining a safe environment. Recognize the importance of continuous diabetes education and self‑learning needs while maintaining current skills and knowledge. Provide educational sessions and tools for the care team. Required Qualifications Bachelor’s degree required. 1‑3 years of related work experience required. Strong interpersonal, patient communication, and patient education skills. Advanced skills with Microsoft applications (Outlook, Word, Excel, PowerPoint, Access) and other web‑based applications; may produce complex documents, perform analysis, and maintain databases. Licensure, Certification, Registration Registered Dietitian license required; Basic Life Support certificate required. Certified Diabetic Care Educator and Specialist (CDCES) required. CPR certification. Preferred Qualifications Experience using diabetes technologies preferred. Competencies Decision Making: make guided decisions and moderate‑complexity recommendations. Physical Requirements Medium work: may require lifting up to 50 lb occasionally, up to 20 lb frequently, frequent walking, standing, bending, twisting, and keyboard use. Pay Range $33.60 – $49.93 per hour (base hourly wage). Vaccination Requirement All staff must be vaccinated against influenza as a condition of employment.
