Child & Family Care Manager - Advocacy & Coordination
Collaborative for Children & Families
Collaborative for Children & Families in New York seeks a Care Manager to coordinate care for children, youth, and families with complex medical, developmental and behavioral needs (0–21). You will develop and implement an individualized Plan of Care in partnership with caregivers and providers, serving as the primary contact to ensure needs and voices are integrated into the plan.
