Location: Dickson City, PA
Coverage Area: Lackawanna, Luzerne, and Wyoming Counties, PA
Employment Type: Full-Time
Work Arrangement: Field-Based (Not Remote)
Compensation: $55,000 - $62,000 annually (estimated, based on per-visit model)

Position Overview
Wheeler Staffing Partners is seeking a compassionate and dependable Licensed Practical Nurse (LPN) to join a home health team delivering high-quality, patient-centered care in the home setting. This field-based role focuses on supporting patient recovery, chronic condition management, and overall health outcomes through skilled nursing visits.


This position operates under a per-visit compensation model , offering flexibility and the opportunity to increase earnings based on visit volume.

Schedule & Compensation Model

• Full-Time Schedule (32 hours/week)
• Paid Per Visit Structure (Single Visit Type Model)
  • Minimum: 32 visits per week
• Typical Schedule: Monday - Friday, 8:00 AM - 5:00 PM (flexibility as needed)
• Mileage reimbursement provided
• On-call rotation required
• Full benefits package available

Key Responsibilities

• Provide skilled nursing care to patients in their homes in accordance with physician orders and individualized care plans
• Monitor and evaluate patient conditions, supporting chronic disease management and post-acute recovery
• Document assessments, interventions, and outcomes accurately within the electronic medical record system
• Collaborate with physicians, RNs, and interdisciplinary healthcare teams to coordinate patient care
• Educate patients and families on medications, treatment plans, and lifestyle modifications
• Travel to patient homes within the assigned territory and manage daily visit schedules effectively
• Participate in on-call rotation as required
• Ensure compliance with all clinical, regulatory, and organizational standards

Qualifications

• Graduate of an accredited practical or vocational nursing program
• Active Licensed Practical Nurse (LPN) license in the state of Pennsylvania (or compact license eligible for PA practice)
• Minimum of 1 year of clinical nursing experience required
• Home health experience preferred

Required Certifications & Licensure

• Current LPN license in Pennsylvania (or compact state eligibility)
• Current CPR/BLS certification
• Valid driver's license, reliable transportation, and active auto insurance
• Ability to travel within assigned territory
