Registered Nurse (RN) – Clinical Assessment & EligibilityMonday–Friday | 35-Hour Workweek | $43.96/Hour | $2,000 Sign-On BonusLooking for a nursing role where you can use your clinical expertise without the demands of traditional bedside nursing?Boston Senior Home Care (BSHC) is seeking an experienced Registered Nurse (RN) to join our Clinical Assessment and Eligibility (CAE) team.This is a unique opportunity for an RN who enjoys assessment, care coordination, working independently, and helping older adults and individuals with disabilities access the services and supports they need to remain safely in the community.Why Consider This RN Opportunity?$43.96 per hour$2,000 sign-on bonusFull-time, 35-hour workweekMonday–Friday, 9:00 AM–5:00 PMNo nights or weekendsNo traditional bedside nursingIndependent, community-based nursing roleOpportunity to use your clinical judgment and assessment skillsCollaborative interdisciplinary team environmentAbout the RoleAs a Clinical Assessment and Eligibility Registered Nurse, you will conduct comprehensive nursing assessments in community and nursing facility settings to determine clinical eligibility for programs and services funded through the Massachusetts Executive Office of Aging and Independence (AGE) and MassHealth.Rather than providing traditional hands-on bedside care, you will use your nursing knowledge and clinical judgment to evaluate individuals' medical and functional needs, determine eligibility for services and nursing facility level of care, and collaborate with healthcare and community partners to support safe, appropriate care.What You'll DoIn this role, you will:Conduct comprehensive nursing assessments for individuals living in the community and nursing facilities.Determine clinical eligibility for Home Care programs, MassHealth-funded services, and nursing facility level of care.Complete clinical assessments and screenings for nursing facility admissions and continued eligibility.Conduct scheduled onsite visits at nursing facilities and collaborate with facility social services teams.Participate in interdisciplinary meetings supporting consumers transitioning from nursing facilities back into the community.Identify individuals who may benefit from community transition programs, including Money Follows the Person (MFP).Collaborate with BSHC nurses, case managers, nursing facilities, hospitals, Adult Day Health programs, home health agencies, and other community partners.Provide clinical consultation to case managers and other BSHC staff regarding nursing and medical issues.Participate in peer review and quality improvement activities.Provide education and training to healthcare and community partners regarding clinical screening and eligibility processes.Develop and participate in educational and outreach activities as needed.What We're Looking ForWe're looking for an RN who is comfortable working independently while also being part of a collaborative interdisciplinary team.The ideal candidate will have:A valid Massachusetts Registered Nurse (RN) licenseStrong clinical assessment and critical-thinking skillsExcellent organizational and independent caseload-management skillsStrong written and verbal communication skillsAbility to collaborate effectively with consumers, families, healthcare providers, and community partnersExperience working with older adults, individuals with disabilities, behavioral health populations, community-based programs, or long-term careStrong computer and documentation skillsA valid driver's license and ability to travel within the communityEducation & ExperienceCandidates must meet one of the following qualifications:Bachelor's degree in Nursing (BSN); orAssociate Degree in Nursing (ADN) with at least one year of clinical experience and/or nursing experience in a community or long-term care setting; orAssociate Degree in Nursing with five years of nursing experience, including two years of community-based nursing or long-term care experience; orDiploma RN with two additional years of community-based nursing or long-term care direct-service experience.Applicable supervisory requirements will be provided in accordance with program requirements.Schedule & TravelSchedule: Full-time, Monday–Friday, 9:00 AM–5:00 PM Hours: 35 hours per week Travel: Up to 25%This position includes travel to consumers' homes, nursing facilities, and other community settings within BSHC's service area.
