Job Title: Home Health Registered Nurse Profession: RN (Registered Nurse) Specialty: Home Health Duration: 13 Weeks Shift: 8 Hour Days Hours per Shift: 40 hours per week Experience: Minimum of 2 years License: Valid driver's license and insurance required Certifications: BLS Must-Have: HCHB - HomeCare HomeBase preferred Experience with patients recently discharged from the hospital Description: Mileage will be paid per GSA guidelines. No pediatric patients. Most patients are recently discharged from the hospital and are at high risk for rehospitalization. Common diagnoses include CHF, COPD, wounds, and some wound vacs. IV-PICC line management is necessary. Experience with respiratory equipment such as vents, CPAP, and BiPAP is required, specifically for patient education. The typical caseload includes an average of 5.25 patients per day. This includes 2.5 points for start of care, 1.5 points for recertification, 1.75 points for resumption of care, and 1 point for routine visits. Care management may include working with home health aides. The schedule is generally Monday to Friday with 8-hour shifts. Flexibility for weekends is necessary, with preferences for Tuesday to Saturday. Holiday coverage may be required. There is no call or standby requirement. This position involves floating requirements within home health only. Dress code includes any work-appropriate scrubs. Charting system utilized is Cerner.
