Responsibilities
- Administer prescribed medications and monitor for adverse reactions in a long-term care population
- Assess patient status, document changes, and communicate updates to the RN and interdisciplinary team
- Provide direct patient care including ADLs, mobility assistance, and daily care routines
- Support wound care, dressing changes, and preventive skin integrity measures as assigned
- Assist with care planning, charting, and ensuring accurate, timely documentation
- Maintain a safe, clean, and comfortable environment; follow infection control protocols and safety procedures

Requirements
- Active LPN license in New Hampshire
- Current Basic Life Support (BLS) certification
- Ability to work 12.5-hour shifts on a 3x12 schedule and meet guaranteed hours
- Prior experience in a long-term care or geriatric setting preferred
- Strong communication and teamwork skills; able to document clearly and collaborate with the care team
- Local candidate preferred; ability to work in the listed setting and comply with assignment requirements
