CNA - LTC/SNF

Job Description

Job Summary:
Provide direct patient care in a long-term care or skilled nursing facility, supporting residents with daily activities and ensuring their safety and comfort. Assist healthcare team members with clinical tasks and patient needs.

Responsibilities:

• Deliver bedside care to residents, including assistance with activities of daily living such as bathing, dressing, and feeding.
• Monitor and document residents' vital signs and overall condition, reporting any changes to the healthcare team.
• Adhere to facility protocols for infection control, safety, and patient privacy.
• Support residents in mobility and repositioning to prevent bedsores and promote comfort.
• Maintain accurate patient records and comply with documentation standards.
• Follow all regulatory and facility-specific policies to ensure high-quality care delivery.

Qualifications:

• Current certification as a CNA (Certified Nursing Assistant).
• Valid state or federal issued ID and proof of COVID-19 vaccination or exemption.
• Ability to work effectively in a team environment and communicate clearly with residents and staff.
• Demonstrate knowledge of clinical care procedures relevant to LTC/SNF settings.
• Exhibit compassion, professionalism, and a strong work ethic.
