Registered Nurse (RN) – Home Health

Albuquerque & Rio Rancho, New Mexico

We are seeking an experienced and compassionate Registered Nurse (RN) to join our growing Home Health team serving patients throughout Albuquerque and Rio Rancho .

This position is ideal for an RN who has a strong background in home health, is comfortable working independently in the field, and has hands-on experience completing Start of Care (SOC) assessments and OASIS documentation .

Minimum Qualifications

Candidates must meet the following experience requirements:

• Active New Mexico Registered Nurse (RN) license in good standing

• Minimum 2 years of total nursing experience

• Minimum 1 year of Home Health nursing experience

• Minimum 1 year of Start of Care (SOC) and OASIS experience

• Strong clinical assessment and documentation skills

• Ability to manage patient care independently in a home setting

• Excellent communication, organization, and time-management skills

• Reliable transportation and ability to travel throughout the Albuquerque/Rio Rancho service area

Responsibilities

As a Home Health RN, you will help patients receive high-quality clinical care while remaining safely and comfortably in their homes. Responsibilities may include:

• Complete comprehensive Start of Care (SOC) assessments

• Complete accurate and timely OASIS documentation

• Perform skilled nursing assessments and visits

• Develop and update individualized plans of care

• Monitor changes in patient condition and communicate with physicians and the interdisciplinary care team

• Provide patient and family education

• Coordinate care with therapists, physicians, caregivers, and other healthcare professionals

• Maintain accurate, timely, and compliant clinical documentation

• Help identify changes in condition and opportunities to improve patient outcomes and prevent unnecessary hospitalization
