Department/Group: Care Ring Clinic Reports To: Care Ring Clinic Supervisor Position Type: Full Time Exemption Status: Exempt Schedule: Monday - Friday 8:00 to 5:00 PM Salary: $67,000 - $70,000 Annually Travel Required: No Summary The Clinical Nurse Coordinator serves as the clinical backbone of Care Ring Clinic, providing essential nursing expertise that elevates the quality and safety of patient care. This role requires an experienced, adaptable nurse comfortable working in a dynamic and evolving environment. The Coordinator focuses on clinical assessment, triage, patient education, preventive care services, and clinical care coordination for complex or high‑risk patients. The Coordinator works collaboratively with the Medical Director, Nurse Practitioners, and medical assistant team to ensure comprehensive, patient‑centered care delivery, enhancing the clinic’s capacity to serve our 6,000‑patient community. Responsibilities Clinical Assessment & Triage Conduct telephone and in‑person triage of patient concerns using clinical judgment and evidence‑based protocols. Assess patient symptoms, provide clinical guidance, and determine appropriate level of care. Make independent nursing decisions regarding urgent vs. non‑urgent care needs. Communicate triage outcomes and recommendations to patients and document appropriately in the EHR. Escalate complex or emergent situations to providers as indicated. Provide clinical oversight to medical assistants regarding patient care concerns. Direct Patient Care & Clinical Procedures Administer immunizations per CDC guidelines and clinic protocols for all age groups. Perform health screenings including blood pressure checks, glucose monitoring, and preventive health assessments. Conduct comprehensive nursing assessments for complex patients prior to provider visits. Perform wound care, including dressing changes and assessment of healing progress. Administer injectable medications as ordered by providers. Collect laboratory specimens, including blood draws when needed. Assist providers with in‑office procedures as needed. Clinical Care Coordination Provide clinical care coordination for patients with complex medical needs or multiple chronic conditions. Conduct post‑hospital discharge follow‑up calls to assess patient status and identify care needs. Coordinate care plans with specialists and other healthcare providers. Identify gaps in care and work with patients and providers to address barriers to treatment adherence. Support transitions of care between different care settings to improve outcomes and reduce readmissions. Monitor high‑risk patients proactively through outreach and follow‑up. Patient & Family Education Provide comprehensive patient education on chronic disease management including diabetes, hypertension. Teach patients about new diagnoses, treatment plans, medications, and self‑management strategies. Counsel patients on preventive health, nutrition, smoking cessation, and lifestyle modifications. Educate families and caregivers on supporting patient care at home. Develop and provide educational materials tailored to patient health literacy levels. Empower patients to take active roles in managing their health. Medication Management Support Review medication lists with patients and identify potential issues with adherence or side effects. Educate patients on proper medication administration, timing, and storage. Assist patients in understanding complex medication regimens. Communicate medication‑related concerns to providers and pharmacists. Support medication reconciliation after hospital discharges or specialist visits. Clinical Quality & Population Health Support quality improvement initiatives and preventive care outreach. Identify patients due for preventive screenings and immunizations. Participate in chronic disease management programs and care coordination initiatives. Document clinical activities accurately and completely in the EHR. Monitor clinical outcomes and contribute to quality metrics reporting. Maintain current clinical knowledge through continuing education. Collaboration & Team Support Serve as a clinical resource for medical assistants and front office staff. Provide clinical guidance and mentorship to the MA team regarding patient care questions. Complete skills competency assessments and checkoffs for medical assistants to ensure clinical proficiency. Collaborate with providers to optimize patient flow and care delivery. Participate in team huddles, case conferences, and quality improvement meetings. Work closely with RMA on administrative care coordination to ensure seamless patient experience. Support clinic operations during high‑volume periods or staff absences. Health Promotion & Community Outreach Support community health education initiatives and wellness events. Participate in health fairs, vaccination clinics, and screening programs. Connect patients with Care Ring’s food pantry and other social support services. Identify and address social determinants of health impacting patient outcomes. Advocate for patient needs and health equity within the healthcare system. Qualifications and Education Requirements Required Current R.N. license in North Carolina or South Carolina. Associate Degree in Nursing (ADN) from an accredited nursing program. Current CPR/BLS certification. Minimum 5 years of clinical nursing experience, preferably in ambulatory care, primary care, or community health settings. Strong clinical assessment and critical thinking skills. Excellent communication and patient education abilities. Proficiency with electronic health record (EHR) systems. Knowledge of evidence‑based nursing practice and clinical guidelines. Ability to work independently and exercise sound clinical judgment. Commitment to serving diverse and underserved populations. Valid driver’s license and reliable transportation. Preferred Bachelor of Science in Nursing (BSN). Experience in community health, primary care clinic, or federally qualified health center (FQHC). Certification in ambulatory care nursing or public health nursing. Experience with chronic disease management and care coordination. Bilingual capabilities (Spanish/English preferred). Knowledge of Medicaid and programs serving patients without health insurance. Experience with telephone triage protocols. Familiarity with social determinants of health and community resources. Teaching or patient education experience. Preferred Skills Advanced clinical assessment skills across the lifespan. Strong organizational and time management abilities. Cultural competency and ability to build rapport with diverse patient populations. Collaborative team player with positive attitude. Flexibility and adaptability in fast‑paced environment. Manual dexterity for clinical procedures and computer documentation. Visual and auditory acuity for patient assessment. Ability to lift and position patients (up to 50 lbs with assistance). Ability to respond quickly to urgent patient care situations.
