What You'll Do

• Provide direct patient care to residents in long-term care and short-stay rehabilitation.
• Administer medications, treatments, and therapies according to plan of care.
• Monitor, assess, and document patient conditions, reporting changes promptly.
• Collaborate with interdisciplinary team members to develop and implement individualized care plans.
• Support patients' goals of rehabilitation and returning home safely.

What We're Looking For

• Valid RN or LPN license and in good standing
• Compassionate, patient-focused, and team-oriented mindset.
• Ability to work day or night shifts, full-time schedule.
• Strong communication and critical thinking skills.
