Job Summary: Tryfacta is seeking a Long Term Care (LTC) for our client in Thorp, WI 54771 . This is a 13 Weeks contract assignment. Position Title: Long Term Care (LTC) Location: Thorp, WI 54771 Duration: 13 Weeks Work Schedule: Variable Day and Evening Shifts (8-Hour and 16-Hour Shifts) Key Responsibilities: Provide direct nursing care to residents in a Skilled Nursing Facility setting. Administer medications and treatments as prescribed. Monitor resident conditions and report changes to the healthcare team. Maintain accurate patient records and documentation. Assist with resident assessments and care planning. Collaborate with physicians, RNs, CNAs, and interdisciplinary care teams to ensure quality patient care. Requirements: Active LPN License required. Long Term Care (LTC) and/or Skilled Nursing Facility (SNF) experience preferred. Strong clinical assessment and documentation skills. Excellent communication and teamwork abilities.
