Key Responsibilities

• Deliver physical therapy treatment services to patients in a skilled nursing facility (SNF) setting under the supervision of a licensed Physical Therapist
• Implement individualized treatment plans as established by the supervising Physical Therapist
• Document patient progress, treatment sessions, and clinical outcomes in accordance with facility and regulatory standards
• Communicate effectively with the interdisciplinary care team regarding patient status and progress

Required Qualifications

• Active Physical Therapy Assistant (PTA) license in the state of Minnesota or eligibility to obtain
• Graduate of an accredited Physical Therapist Assistant program
• Experience working in a skilled nursing facility (SNF) setting preferred
• Current CPR/BLS certification

Preferred Experience / Skills

• Prior experience in a SNF or long-term care environment
• Familiarity with electronic medical records (EMR) documentation systems
